<p>These powerful muscles surrounding your hips are vital for skiing. Reducing pressure on your back and creating stability around your pelvis, strong gluteals work to reduce the risk of damage to you knees once allowing you to turn effectively on the snow.</p>

So we thought it would be good to explain a common <a href="https://www.comforthealth.co.uk/running-injuries/">running injury</a> and the best way to look after it to be able to keep training.</p>
<h3>Metatarsal Stress Fracture</h3>

<h3>What is it?</h3>
<p>A <a href="https://www.runnersworld.com/health-injuries/a19596239/stress-fracture-symptoms-treatment/">stress fracture</a> is an over use injury, common at the metatarsal bones in the foot, navicular bone and the tibia. Less common sites are around the hip, pelvis, sacrum and lumbar spine. A stress fracture is developed following repeated movements that stress the bone, causing micro trauma from sub maximal loading. The appearance of a stress fracture brings pain on movement, which increases as the movement is repeated, but eases with rest.</p>
<p>The pain is characterised as being very local (pain when the bone is touched). There would be little to no pain until you have hit the site of the stress fracture. The metatarsals are prone to stress fractures in runners, due to the forces places through this area.</p>
<p>&nbsp;</p>
<h3>Early warning signs</h3>
<p>&#8211; Pain on activity, at a specific site within the foot, usually over one or two metatarsals.</p>
<p>&#8211; Symptoms ease with rest.</p>
<p>&#8211; Pain doesn&#8217;t go away as activity increase, and, if anything, gets worse.</p>
<p>&#8211; The area may also be swollen.</p>
<p>&nbsp;</p>

<h3>Progression of the injury</h3>
<p>Usually pain will limit you doing any further damage. However, for those who continue, the pain will eventually get worse at the site and this pain will spread making the injury worse, prolonging your healing time.</p>

<h3>Self Assessment</h3>
<p>Run your hand over the foot until you feel an area that is clearly in the most pain. Press firmly about three inches away from the area of pain, onto the same bone.  Gradually press along the bone getting closer to the site of pain. If you feel little or no pain until you are directly over the injury site, or only slightly past it, but immense pain directly over the injury, this is typical of a stress fracture.</p>

<h3>Treatment</h3>
<p>Rest is key for this injury! In the worst cases you may have to wear an aircast boot to protect the foot for 6 weeks. Physiotherapists can usually diagnose this injury effectively. Stress fractures are only visible when they start to heal as you can see the bone that is lead down for healing (callus) around the fracture site. Therefore, X-rays should be taken at least four weeks after the onset of symptoms otherwise the fracture might not show up.</p>

<h3>What to expect from a Physiotherapist</h3>
<p>The practitioner will listen to your story as this usually provides all the clues needed to make a diagnosis. They will then carry out an assessment of the foot. This includes, assessing range of motion, strength tests, and applying pressure to the metatarsal bones in your feet, looking for points of pain.</p>
<p>Leading on from the diagnosis a plan will be formulated. Usually this includes a six week rest period, but that doesn&#8217;t mean you can drop straight back into running at the end of the six weeks. An exercise program to keep up some lower limb conditioning to assist the smooth return to running is highly recommended.</p>

<h3>What is it?</h3>
<p>The knee has a major articulation between the femur (thigh bone) and the tibia (shin bone). These bones meet with surfaces that don&#8217;t match up particularly well. To improve the joint congruency, two horseshoe-snapped pieces of cartilage called menisci are positioned at the end of each bone.</p>
<p>The menisci not only improve congruency of the joint, but act as shock absorbers during activity.</p>
<p>These are fragile and with age become more prone to injury. The biomechanics of the knee joint play a huge part in the maintenance of the menisci, as a perfectly aligned knee will be much less likely to damage the cartilage.</p>
<p>There is no evidence to support the notion that running alone speeds up the degradation of these structure or the knee itself. However, it is logical to accept that poor alignment will damage the surfaces and everything in between at a faster rate (Hobrough, 2016).</p>
<p>Cartilage can tear (acute injury) or break down (chronic injury) over time. Acute injuries usually occur with a twisting motion as the two bones provide both a compressive and rotational force to the cartilage. This is sufficient to cause a tear within the material.</p>

<h3>Early Warning Signs</h3>
<p>&#8211; From time of injury swelling is delayed by 7 -12 hours</p>
<p>&#8211; Pain in straightening the knee</p>
<p>&#8211; Joint line tenderness</p>
<p>&#8211; Knee gives way, or feels unstable</p>
<p>&#8211; Knee locks</p>

<h3>Common reason for injury</h3>
<p>The injury can be acute (sudden onset) from a rugby tackle or falling over skiing. Or can come on over a period of years, from gradual break down through a biomechanics fault with the alignment of the knee.</p>
<p>&nbsp;</p>
<h3>What to expect from a Physio appointment</h3>
<p>Your Physio will carry out a variety of tests. These tests can sometimes produce the same pain you have been experiencing. We try to keep pain to a minimum but it is important that we achieve a robust diagnosis.</p>
<p>The likely outcome is a referral for an MRI scan. However, your Physio can give you some good exercises to do that can both help with the alignment and also prepare you for surgery.</p>

<h3>Treatment</h3>
<p>Surgery is normally required to clean out any torn menisci within your knee. This is done by key hole surgery, which enables a swift recovery. However, the issue still remains that if your menisci became worn due to a chronic injury, then whilst an operation is one route, physiotherapy is now thought to be the leading treatment.</p>
<p>Specific rehabilitation and manual therapy are key aspects when dealing with a meniscus injury. The goal of any rehabilitation would be to correct any biomechanics issues that may have lead to the injury. As well as a specific strength program to reduce the chances of re-injury.</p>
